For any verification of employment for State of Utah employees, please contact the Employee Resource Information Center (ERIC) at 801.957.9390 or you can fax us at 385.465.6054, or email to erictransactions@utah.gov.

In order for the ERIC to complete a written verification of employment for an employee, we must have at least the last four digits of the employee’s social security number, and the request must be an authorization form that is signed by the employee. Information we can provide in a written verification of employment:

• Dates of Employment
• Salary (including hourly rate, current and prior yearly salary)
• Average hours worked
• Date of last pay increase
• Amount of last pay increase

FOR W-2 FORMS
Duplicate W-2's can be requested after March 1st directly from the Division of State Finance. To request a W-2 send an email to payroll@utah.gov, you MUST include the following in your email:

• Your full name
• The W2 year you are needing
• The last 4 digits of your social security number
• Your date of birth
• Your current full address
